The Care Planning Team of the Massachusetts Advisory Council on Alzheimer’s Disease and All other Dementias has released a new toolkit for people living with dementia, dementia care partners, families, community-based service providers, and health care providers. The purpose of the toolkit is to facilitate the development of person-centered dementia care plans that support living well with dementia.
The Advisory Council will be hosting a webinar on Thursday, March 27 from 12:00 pm – 12:40 pm for community-based service providers and health care providers. Click here to register.
