Don’t Get Left Out in the Cold: Preparing Documentation
& IDT Systems for QRP & VBP Validation Audits
Tuesday, December 16, 2025
12:00 pm – 12:45 pm
VIRTUAL
Seminar Description:
CMS is expected to expand QRP and VBP validation audits in 2025, putting up to 2% of a facility’s annual Medicare revenue at risk. Strong, consistent documentation is essential to accurately show resident acuity, skilled services, and outcomes. Breakdowns in documentation, communication, or assessment alignment can result in avoidable penalties and weaker quality scores.
This session offers a practical review of how documentation affects QRP and VBP performance, what CMS will look for in audits, and common issues that increase compliance risk. Attendees will learn strategies to strengthen IDT processes, improve the clinical narrative, and ensure documentation is complete, defensible, and audit-ready.
Participants will leave with clear steps to stay compliant, support quality outcomes, and safeguard reimbursement amid increasing federal oversight.
Objectives:
Attendees will:
•Understand what QRP and VBP validation audits require—what documentation is needed, when it’s due, and what triggers a review.
•Recognize how missing or inconsistent documentation and IDT communication can lower QRP and VBP scores and affect overall quality.
•Identify strong, IDT-driven documentation practices to clearly show resident acuity, skilled services, and clinical outcomes.
Presenters:
•Maureen McCarthy, President and CEO of Celtic Consulting, LLC
