The Transitions from Acute Care to Post-Acute Care (TACPAC) Task Force was established under Section 25 of Chapter 197 of the Acts of 2024, the LTC Reform Law, to study and propose recommendations to address acute care hospital throughput challenges and the impact of persistent delays in discharging patients from acute to post-acute care settings. The work of the Task Force focused on identifying system barriers, advancing innovative solutions, and promoting timely, appropriate care transitions. The Task Force was chaired by Assistant Secretary Joanne Marqusee, acting as the designee of the Secretary of Health and Human Services, and was comprised of a diverse panel of public health professionals, law enforcement, first responders, and experts in health care administration and finance. Tara Gregorio, President of the Mass Senior Care Association was an appointee of the Task Force. On July 31, The Task Force concluded its work with the submission of its report to the Legislature.
