JOB TITLE: CARE PARTNER
FLSA STATUS: NON-EXEMPT
DEPARTMENT: PINES EDGE SKILLED NURSING
REPORTS TO: DIRECTOR OF NURSING
POSITION SUMMARY (Scope/Purpose)
The Certified Nursing Assistant who will provide care for residents and administer necessary unit tasks and functions in compliance with North Hill’s policies, procedures and standards. The individual will partner with residents to provide resident centered care including ADL’s and the preparation and serving of meals.
JOB SPECIFIC DUTIES:
• Provide resident centered care including ADL’s, bathing, dressing, grooming, nail care, shaving, and bowel and bladder functions.
• Assist in transporting residents to/from appointments, activity and social programs
• Respond to resident requests for support and uses communication systems appropriately
• Leads and participates in small group activities
• Report all changes in resident condition to the nurse and completes necessary documentation in a timely manner. Respond to all emergencies.
• Advocate for and understands resident personal preferences and provides support/care in a manner that respects resident rights, choices, confidentiality, privacy and protects them from abuse.
• Complete necessary documentation for ancillary services and supplies to ensure tracking and correct billing can occur. Utilized electronic health documentation appropriately.
• Assume Coordinator responsibilities within the household on a rotating basis.
• Prepare, cook, serve and complete after meal clean-up for resident meals. Ensure food handling guidelines. Follow appropriate meal plans for resident preferences to include food allergies, diet and texture restrictions.
• Complete food/equipment checklists and logs as required.
• Provide support with light housekeeping and laundry as assigned or as needed. safety requirements and exposure procedures. Follow all Emergency Management Plan, including the Emergency Dining Plan and Life Safety policies and procedures and actively participate in fire and evacuation drills.
• Ensure that all team members speak in the primary resident language in all resident care areas.

Supervisory Responsibility
This position has no supervisory responsibility.
Work Environment
Will be subject to frequent interruptions. May be asked to work beyond normal working hours and to work in other areas temporarily, when necessary. May be subject to call-backs during emergency conditions (e.g., severe weather, evacuation, post-disaster).
Physical Demands
Must be able to move freely throughout the workday. Must meet the general health requirements set forth by the policies of North Hill. Must possess adequate vision/hearing (corrected or not) to immediately assess the surroundings to ensure Resident safety and to adequately address issues that arise during workday. Must be able to work with moderate to loud noise level. Must be able to lift up to 50lbs up to 25% of the time, 100lbs 0% percent of the time.
Position Type/Expected Hours of Work
This position regularly requires weekend and holiday work.
Required Education and Experience
High School Diploma or equivalent.
Certified Nursing Assistant certification in the State of Massachusetts
Ability to read, write and understand English
2 years’ experience as a C.N.A. preferably in a long term care setting
