SUMMARY
Provides skilled physical therapy services in the center consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives.
EDUCATION
Minimum of a Master's Degree from an accredited Physical Therapy program for RPT.
Minimum of an Associates Degree for PT/PTA.
EXPERIENCE
Prefer a minimum of one year of experience in a therapist capacity in a skilled nursing facility setting or other related rehabilitation setting.
QUALIFICATIONS
Licensed as a Physical Therapist in the state(s) of practice.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS
Must hold the knowledge of basic computer literacy
Provides physical therapy in accordance with physician's orders and accepted professional
Performs patient assessments and evaluations to determine functional level and establish a therapeutic plan of
Develops an overall therapy plan of care to include: treatment and medical diagnosis warranting treatment , the planned duration and frequency of therapy to be provided, a minimum of long term goals and discharge plans; per standards of practice, and established
Utilizes standardized assessments upon the development of the plan of care to validate the need for skilled therapy services, and demonstrate repetition of this standardized assessment to reflect the ongoing need for skilled therapy and the patient 's progress from received
Completes all documentation required and in accordance with state and federal regulations and
Documents observations, patient 's response to treatment, treatment program s, degree of progress and notable changes in patient's condition within required time
Completes all billing required on the date of the provided treatment, and in accordance with all state and federal regulations and
Communicates and coordinates patient's care with other members of the patient' s health care
Supervises physical therapy assistants in accordance with regulations.

Assist the patient and/or family to procure needed equipment or supplies prior to discharge from
Utilize appropriate infection control practices including hand washing, standard precautions and personal protective equipment per facility
Hold the skills of and demonstrate strong time management and organization skills, as well as seamless communication
Maintain a positive attitude within compliance to the company' s code of conduct.
Participates in staff meetings and patient conferences as directed by the Director of
Provides proof of attendance at mandatory in-services or training programs per center
Maintain skills and education by attending continuing education and in-service program
Responsible for meeting facility designated productivity expectations as directed by the Director of Rehabilitation.
Other duties as required.
