Summary
Responsible for the assessment of spiritual and religious needs of the residents and family and arranging for the provisions of spiritual and religious support in accordance with the wishes of the resident and family. Supports the Mission Statement of the organization.
Education
A degree in ministry, psychology, theology or counseling from an accredited college, university or divinity school is recommended.
Professional experience in a religious agency or institution may be considered in lieu of a formalized education.
Experience
At least one year of supervised experience in a medical, educational or religious agency or institution, Hospice/Home
Heath experience preferred.
CPR Certified
Minimum of five years' progressive experience such as working with death and dying, providing comfort in an interfaith setting to individual/family/caregiver.
Experience in recruiting, training and supervising volunteers preferred.
Job Functions
Will complete a spiritual assessment to determine the spiritual and religious needs of the resident and family along with arranging the appropriate provision of spiritual and religious services
Will be a liaison between the resident/family and their community of faith
Will develop and document a spiritual support plan
Will arrange for counseling services to be provided as needed
Will meet with Chaplin/Pastoral Care Coordinator of partnering hospice groups. Become the liaison with each hospice and to ensure the residents and families’ spiritual needs and wishes are met.
Will arrange biannual memorial remembrance celebrations.
Will run resident groups and provide 1:1 resident meetings for both the SNF and Adult Day Health..
Will assist and coordinate bringing residents to mass/communion service daily.
Will manage priest coverage for mass 6 days a week.
Will recruit and train volunteers from the community and schools.
Will build community relationships by providing education about the St. Francis mission.
