This job description, defining qualifications, primary (essential) and secondary (non-essential) functions, responsibilities and duties, is intended as a guideline for the employee and manager. ESSENTIAL FUNCTIONS:
Management Function Requirements:
Supervises and evaluates all direct resident care and initiates corrective action as necessary.
Provides clinical supervision of nursing assistants providing direct resident care.
Follows staffing guidelines and makes nursing assignments according to qualifications of staff and characteristics of the resident population.
Assumes responsibility for compliance with federal, state and local regulations within the assigned unit.
Counsels nursing assistants and recommends disciplinary action, when necessary, to the Director of Nursing Service.
Instructs nursing assistants in proper, preventive safety measures and use of adaptive equipment to meet the needs of each resident.
Controls supplies and equipment within the unit.
Collaborates with interdisciplinary team to ensure that high quality resident care is maintained.
Prepares nursing assistants' assignments and explains all pertinent resident care.
Supervises the application and removal of all postural supports and restraints.
Supervises distribution of nourishments.
Orders, receives and documents medication received according to facility procedure.
Supervises response to residents' calls for assistance and responds to call lights as necessary.
Insures that all physician renewal orders and physician documentation is timely and appropriate.
Plans and conducts unit staff meetings as needed and records problems and recommends action. Suggestions for change are referred to appropriate person.
Patient Care Requirements:
Obtains report from nurse he/she is relieving and records sufficient information to implement appropriate follow-up action as necessary.
Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary.
Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action.
Identifies resident problems and emergency situations, and initiates immediate "life saving" measures within the scope of nursing practice and in accordance with advance directives.
Administers and documents medications, enteral nutrition, and treatments per the physician's order and accurately records all care provided and observations daily.
Performs comprehensive assessment of each resident.
Documents comprehensive assessment of each resident with review by Registered Nurse.
Develops an accurate comprehensive care plan based on each resident's comprehensive assessment.
Assesses on a weekly basis the skin condition, stage, size (measurements), site, depth, color, drainage and odor of condition.
Obtains physician's orders and any revisions for all treatments and medications needed.
Renders immediate assistance to any resident in need.
Provides intravenous therapy according to facility policy and procedures.
Provides respiratory care according to facility policy and procedures.
Assists physician during treatments and examination of residents.
Checks meal trays to assure proper diet is served and residents are assisted as necessary.
Observes, evaluates and documents all changes in residents conditions.
Notifies physician, when necessary, of changes in residents conditions, weight, medication errors, medication not available, abnormal laboratory/X-ray results and any other unusual observations.
Administers PRN medications and treatments as necessary.
Documents resident care provided and resident's response or lack of response to care provided.
Reports changes in residents' conditions to a physician, Director of Nursing Service and responsible party promptly and to take follow-up action as necessary.
Follows facility admission procedures and communicates resident admission to other departments, physician and ancillary service providers.
Knowledgeably and competently performs basic nursing skills.
Follows facility discharge/transfer procedures and coordinates discharge/transfer with other departments, physician and ancillary service providers.
Assesses residents' needs and initiates development of individualized plans of resident care.
Provides direct resident care and gives instruction to nursing assistants related to resident care.
Listens to residents', families, or physicians' concerns and initiates corrective action and/or reports concerns to Nursing Supervisor.
Safety & Health Requirements:
Identifies safety hazards and initiates corrective action immediately.
Observes infection control policies and procedures.
Assumes responsibility for the safety of residents under his/her supervision.
Observes all facility fire, safety, and evacuation policies and procedures.
Counts narcotics/signs for count, checks emergency kits and exchange keys.
Maintains and monitors medication room and med cart in clean, safe and secure manner.
Maintains utility room and treatment cart in clean, safe and secure manner.
Records and destroys non-countable meds with another nurse.
Follows facility procedures related to control of equipment and supplies within the unit. Assures that work assignment areas are clean and equipment and supplies are stored before leaving care area on breaks and at end of shift.
Professional Development Requirements:
Participates in facility education programs and coordinates student activities with the Staff Development Coordinator.
Attends staff meetings, in-service classes and committee meetings as assigned.
Personally obtains continuing education in the field of nursing relating to specific needs of resident population.
Comes to work in a clean, neat uniform and consistently present an appropriate professional appearance in accordance with facility dress code.
Comes to work as scheduled and consistently demonstrate dependability and punctuality.
Accepts assigned duties in a cooperative manner.
Documentation Requirements:
Reviews and revises resident care plan when documenting progress notes.
Assists in maintaining of residents' clinical records.
Monitors all forms of documentation and edits appropriately. i.e. monthly physician orders, treatment sheets, etc.
Completes assigned sections of the MDS.
Completes monthly nursing summary notes as assigned.
Documents individual nursing progress notes according to needs and condition of residents.
Completes documentation related to the residents' discharge/transfer.
Reviews and revises resident care plans as necessary and as required by regulations.
General Requirements:
Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

Interprets existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.

1. Graduate of an accredited LPN school of nursing.
2. Current license in Massachusetts as LPN.

Persons who perform these duties are not called upon to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way as part of their employment. Tasks that involve handling of implements or utensils, use of public or shared bathroom facilities or telephones, and personal contacts such as handshaking are Category III tasks.
