Project Re-Engineered Discharge (RED) is an evidence-based intervention that provides a systematic approach to prepare patient and caregivers for a successful discharge back to the community. Through a three-year Special Innovation Project, Healthcentric Advisors has partnered with hospitals, nursing homes, home health, and local aging services throughout Southeastern Massachusetts to enhance discharges back to the community, impacting nearly 4,500 patients.
On Thursday, August 22nd, from 10 am to noon, Healthcentric Advisors QIO will convene a meeting to learn about the outcomes of this special project, understand how they connect to provider quality reporting/measures, and discover best practices to position your organization provide to succeed in an ever-changing health care environment, all while providing exceptional care transitions.
