MassHealth issued an FAQ to address questions that have been submitted by facilities in regard to completing the Nursing Facility Ownership Disclosure Form. In addition, MassHealth is also further extending the filing deadline to August 16, 2026 to allow facilities more time to complete the filing.
MassHealth has reinforced Mass Senior Care’s recommendation to members that the form be completed with “good faith” responses in accordance with the available instructions on the NF Ownership Disclosure Form Instructional Video and the FAQ. In the event that MassHealth requires further information upon the filing, MassHealth will reach out to individual facilities.
