Dementia Special Care Unit Disclosure Due by March 1st

  • Dementia Special Care Unit Disclosure Due by March 1st

Annual Dementia Special Care Unit (DSCU) Disclosure statements must be filed with the Department of Public Health no later than March 1st. Disclosure statements must be submitted using the Health Care Facility Reporting System (HCFRS).

Please note that once completed, the disclosure form must be scanned and then attached into HCFRS. Please do not fax or mail your disclosure form to the Department.

Facilities which advertise or hold themselves out to the public as a provider of specialized care to persons with dementia that do not file a disclosure form in a timely manner may be subject to fines under MGL Chapter 111, section 73.
