The Massachusetts Department of Public Health would like to remind all licensed adult day health programs, non-ambulatory surgical center clinics, nursing homes, and rest homes that the deadline for the mandatory reporting of healthcare personnel influenza data is April 15, 2018 at 5:00 pm. Beginning with the 2017-1018 flu season, all licensed adult day health programs, non-ambulatory surgical center clinics, nursing homes, and rest homes must submit influenza vaccination reports through the web-based application SurveyMonkey.® Please review the Detailed Directions for 2017-2018 Data Reporting for instructions on submitting the data from your facility and guidance sent earlier this reporting season. You may access the SurveyMonkey application here to submit the data for your facility. Please note paper versions of the survey will not be accepted.
