  • Commonwealth Corporation Releases Case Study on Care Transitions Education Project

Commonwealth Corporation has released a case study on the Care Transitions Education Project (CTEP) developed and administered by Mass Senior Care Association to equip frontline nurses with the foundational knowledge and skills needed to lead and improve care transitions and reduce costs. The project was overseen and received funding from the Commonwealth Corporation through the Health Care Workforce Transformation Fund, which supports training programs that address workforce skill needs idenĀ­tified by health care providers to improve patient care and reduce costs.
