JOB TITLE: LICENSED PRACTICAL NURSE
FLSA STATUS: NON-EXEMPT
DEPARTMENT: PINES EDGE SKILLED NURSING
REPORTS TO: DIRECTOR OF NURSING
POSITION SUMMARY (Scope/Purpose)
The Licensed Practical Nurse (LPN) provide direct nursing care in accordance with standard nursing practice and in accordance with North Hill standards of care. The LPN will assist the Nursing Supervisor and Director of Nursing in conducting clinical rounds and directly collaborate with Resident Care Partner regarding resident care needs. The LPN will report directly to the Director of Nursing (DON).
JOB SPECIFIC DUTIES:
• As Primary Nurse, coordinate the interdisciplinary care plan providing person centered care, to include all documentation and required assessments based on nursing standards of practice.
• Understand and demonstrate the importance and use of person centered care as well as coaching team members to utilize person centered approach to care and language.
• Administer medications per Pines Edge policy and procedures and state regulations. Ensure medication documentation is accurate and completed in a timely manner.
• Ensure medication and treatment supplies are available in resident rooms.
• Responsible for completion of Medicare documentation according to the Medicare Guidelines.
• Monitor and collaborate with Resident Care Partners and coach them to ensure resident care meets North Hill standards.
• Utilize the SBAR form for medical practitioner communication. Ensure timely notification to medical practitioner and responsible party of changes in resident’s condition.
• Respond to Resident Care Partner use of the “INTERACT” form indicating change in Resident condition.
• Update individual resident care plan in collaboration with the resident, responsible party and interdisciplinary team. Ensure all communication is carried over from shift to shift.
• Manage physician orders electronically. Manages physician visit compliance.
• Complete necessary documentation for ancillary services and supplies to ensure tracking and correct billing can occur.
• Recognize and report all allegations of resident abuse and/or misappropriation of resident property PROMPTLY to DON or Administrator to comply with regulation to report to DPH within 2 hours.
• Complete accident/incident reports as necessary on a timely basis. Notify family/ physician according to regulations and standards.
• Ensure that all team members speak in the primary Resident language (English) in all resident care areas.
• Actively participate in facility surveys made by authorized governmental agencies.
• Assist in the orientation program welcoming new team members to Pines Edge.
• Provide support in the houses as necessary. This includes kitchen safety, light housekeeping and laundry as needed.
• Follow safety requirements, to include Emergency Management Plan and established Infection Control and Universal Precautions policies and procedures.
• Completes required educational activities as assigned.

Will work in a well-lighted and ventilated area with frequent interruptions. May be asked to work beyond normal working hours, and in other positions temporarily, when necessary. May be subject to call-backs during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). May be asked to work beyond normal working hours and to work in other positions temporarily, when necessary.
Physical Demands
Must be able to move intermittently throughout the workday. May be necessary to assist in the evacuation of residents during emergency situations. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must be able to stand/walk for 75-100% of the time. Must be able to uses hands, reach with hands, climb and or balance 50-75% of the time. Must be able to stoop, kneel or crawl, climb stairs, talk, hear, taste and smell Must be able to work with moderate to loud noise level. Must be able to lift up to 10 lbs 75%-100% of the time, 25lbs 50-75% of the time, 50lbs up to 25% of the time, 100lbs 0% percent of the time.
Position Type/Expected Hours of Work
This position may require long hours and includes weekends and holidays.
Travel
No travel expected in the position
Required Education and Experience
• LPN degree.
• Current CPR and AED certification required.
• Minimum of 2 years nursing experience in a hospital, long term care facility or other related health care facility.
Preferred Education and Experience
Training in rehabilitative and restorative nursing practices, preferred.
