Notre Dame Health Care, the operator of a five-star rated rehab and long term care center, assisted living residence, memory care units, Medicare licensed hospice, palliative and pediatric care programs, care management program, and an educational center, all located in Worcester, MA seeks a Director of Activities and Memory Care for its Long Term Care Facility.
Responsibilities:
• Compile and maintain an activity schedule, available projects, crafts, resources, etc that meet the individualized needs of residents. Assure that a current resident activity schedule is maintained for each patient.
• Develop and maintain good working rapport with other departments within the facility, resident families, and outside community health, welfare, and social agencies, to assure that activity programs can be properly administered to meet the needs of the residents.
• Work as a member of the Interdisciplinary Team to complete required assessments and/or documentation.
• Develop resident care plans with the members of NDHCC’s Interdisciplinary Team. Execute care plan interventions, documenting progress toward goals in a timely manner.
• Serve on various committees of the facility as may be appointed by the Administrator. Participate and assist in departmental studies and projects as assigned.
• Hire, supervise, and train Activity Aides and ensure the delivery of high quality activity/social programs are adhered to.
• Assist in developing and implementing policies and procedures for identifying the spiritual, social, recreational and emotional needs of residents.
• Attend activities and perform assistance with socials, crafts, projects, etc., as necessary.
• Provide consultation to member of our staff, community agencies, etc. in efforts to solve the needs and problems of the resident through the development of activity/social service programs.
Requirements:
Certification as an Activities Director or Recreation Therapist granted by an organization recognized by the Commonwealth of Massachusetts Department of Public Health required.
Must meet one of the following position requirements:
1. Three to Five (3-5) years experience as an Activities Director in a long-term care or rehabilitation setting or
2. Licensed qualified Occupational Therapy Assistant (COTA/L) with at least one year experience in social/recreational program in a skilled nursing facility or
3. One to three (1-3) years experience as a an Activities Director in a long-term care or rehabilitation setting, and certification as a
4. Memory Care Practitioner (DCP) granted by an organization recognized by the Commonwealth of Massachusetts Department of Public Health or Alzheimer’s Association.
5. Excellent oral and written communication skills in the English language with the ability to communicate effectively, in writing, as well as verbally required. Ability to plan, organizes, develop, implement, and interpret the programs, goals, objectives, policies, and procedures of the Activities Department.
