2021 Annual Disciplinary Summary Action Report – Board of Registration in Medicine

Under M.G.L. c. 111, § 53B and M.G.L. c. 111, § 203, all health care facilities, clinics, and nursing homes are required to submit a Health Care Facility Disciplinary Action Annual Summary Report (“HCFD-3”) to the Data Repository Unit (“DRU”) of the Board of Registration in Medicine (“the Board”) each year. The DRU is part of the Board’s Legal Unit and processes all statutory reports sent to the Board from mandated reporters.

The HCFD-3 form should include all disciplinary actions taken by your facility for the calendar year 2021, i.e., January 1, 2021, through and including December 31, 2021. Report only disciplinary actions or actions for which you filed (or were required to file) a Health Care Facility Disciplinary Action Initial Report (“HCFD-1”) in 2021. Submit the signed 2021 HCFD-3 even if your facility took no disciplinary actions during the year.  A completed HCFD-3 must be submitted no later than January 31, 2022, to borim.statutory.reports@mass.gov. Documents to complete the reporting are below:

  • 2021 HCFD-3 form (.doc) or
