Reducing Unnecessary Rehospitalizations Workshop
THURSDAY, MARCH 19 FROM 8 A.M. - NOON

Location: Hilton Garden Inn Boston/Waltham
450 Totten Pond Rd, Waltham, MA 02451

Attendees: Administrators, Directors of Nursing, Clinical Team Members, Corporate Nursing Support Staff, and anyone involved in nursing or quality leadership.

An interactive workshop focused on reducing avoidable rehospitalizations and their impact on residents, families, and staff. Participants will receive an overview of core practices and medical management strategies designed to support improved outcomes and care coordination, including:
   -Effective communication and care coordination
   -Preventive care and early identification programs
   -Advance care planning and goal setting
   -Establishing a structured risk assessment process
   -Staff education and training strategies
   -Approaches to reducing polypharmacy

This workshop is designed to provide practical tools, education, and best practices to support facilities in improving quality outcomes and reducing avoidable hospital transfers.

The Massachusetts Senior Care Association has qualified for “deemed status #NH-09015-007” from the Board
of Registration of Nursing Home Administrators for Administrators. This program also meets continuing
education requirements for Nurses in Massachusetts. All paid attendees will receive a certificate of attendance.

Cancellation Policy:
Mass Senior Care must be notified in writing no later than 48 hours prior to the start of the seminar for a
refund, minus a $25 processing fee, which will be refunded in the manner in which the payment was made. “No
Show” attendees will be billed at the full registration fee. Cancellations should be emailed to
