Nursing facilities must submit the Final Compliance report by July 31, 2018. The URL to login to the webform is: https://www.uenter.org/nf/. If facilities are unable to login to the webform for any reason, please email NFdirectcare@umassmed.edu and they will work to reset your account.

This EXCEL spreadsheet is designed to mimic the webform. It is for informational purposes only but it should give Registered Users an opportunity, if you choose, to do a preliminary analysis before entering the data on the electronic webform.
The methodology remains the same with the January 2016 through June 2016 period being the base period and the rate period now being July 2017 through June 2018. Please note that while the methodology remains the same, EOHHS/UMASS will be collecting reporting information on overtime hours and dollars for this filing. EOHHS/UMASS recommends web browsers Google Chrome or Mozilla Firefox; the webform is not compatible with Internet Explorer or Microsoft Edge.
